Carers Support in GP Surgeries

Hi, I am Alison Hill, I work at The Carers Hub and I am the Health Liaison Worker for Nottingham and Nottinghamshire.

My role is to help Carers understand how their GP Practices can support them as Carers and their loved ones.

  • Is to work with all GP Practices across Nottingham and Nottinghamshire and wider Health Services to raise awareness of unpaid Carers
  • To provide Carer awareness training to GP Practice Teams and other Health Professionals to help them identify and support more Carers within their Practice.
  • To also support GP Practices to have Carer Registers and Carer Champions.
  • Provide all GP Practices and wider Health Services with local Carer Service information and other useful resources which they can share with Carers and Patients to increase identification, Carer registration in GP Surgeries and access to support.

A Carers Register is a secure list on the GP computer system that shows the Surgery Team which Patients are providing unpaid care to someone with a long – term health condition and care needs, this could be a child and/or Adult who is a friend or related to them.

  • Your GP Surgery offer you useful Carers information and support
  • Your GP Surgery offer you priority flu vaccinations
  • The GP Surgery try to offer flexibility with your appointments to support your caring role and maintain your own health needs.
  • The Surgery to gain consent to share information and make referrals for support for you and your loved one
  • Ask the GP Surgery that you attend as a Patient yourself.
  • At the GP Surgery you attend ask someone on Reception or a Medical Professional for a registration form.
  • Fill out the registration form.
  • If your loved one attends a different GP Surgery you must also make their GP Surgery aware of your caring role so this can be put on the Patient’s notes.

Please note that you must request to be put on the Carers Register if you want this, it is not something that the Surgery Team can do without your consent.

Every GP Surgery has a Carers Register and a Carers Champion as required by the NHS.
Each Surgery has a Carers information board in the waiting area where it will tell you who the Carers Champion is and give you up to date local Carers information. A Carers Champion is a voluntary role.

  • Promote the importance of GP Carer registration for Young and Adult Carers
  • Listen to Carers needs and signpost them to local organisations for support
  • Help to coordinate actions for Carers and their loved ones within the Practice
  • Help to ensure all Carers are identified, registered and supported by the Practice
  • Provide information and resources to Carers
  • Promote Carers awareness across the Practice Team and with Patients.
