Diabetes Awareness Event

Join Nottingham West Social Prescribing for their Diabetes Awareness Event.
The event is designed to support patients living with or at risk of diabetes. The session will be hosted by members of the Nottingham West Primary Care Network including the PCN Dietitian, Health and Wellbeing Coach and Nurse.
